Therapy Benefits Full-Time

Company Paid Benefits:

• $500 reimbursed per year for continued education- must be pediatric specific (CPR
  included)
• Reimbursements of tuberculosis tests (Up to $30.00), CA Therapy Board License, Costco
  & AAA memberships (basic membership)
• 7 Paid Holidays (6 hours paid for each day)
• 2 Floating Holidays: Birthday Holiday & Anniversary Holiday (6 hours), must be taken
  30 day before or after eligible date.
• Paid Vacation Time (60 hours, based upon accrual and tenure)
• Paid Sick Days (30 hours per year, based upon accrual)
• 401K Plan: 4% company paid contribution of annual salary (eligible after 1 year of
  employment, please see 401K Plan Summary for Eligibility)

Medical Benefits:

Health Insurance Minimum 30 +hours/Treatments – eligible after 60 days of continuous
employment:

• Kaiser Silver Plan: Employee only 100% paid by the Company
  o Upgrade: United Health Care (HMO, PPO), Kaiser (Gold or Platinum): Employee
  pays the difference based on Kaiser Silver Plan rate
• Dental Guardian (DHMO): Employee only 100% paid by the Company
  o Dental Guardian (PPO): Employee pays the difference based upon Guardian
  (DHMO) rate.
• Vision Guardian VSP: Employee only 100% paid by the Company
• Hartford Life: $20,000 Employee only 100% paid by the Company
  o Hartford Life Upgrade: Employee can add an additional amount, 100% paid by
  the Employee
• Supplemental: TransAmerica Accident Insurance & Critical Illness Insurance, 100% paid
  by the Employee

*Coverage for Dependents is available and are 100% paid by the Employee

Administrative Staff Benefits Full-Time

Company Paid Benefits:

Full-Time Benefits 40 hours– eligible after 90 days of continuous employment:

• 7 Paid Holidays* (8 hours paid for each day)
• 2 Floating Holidays: Birthday Holiday & Anniversary Holiday (8 hours), must be taken 30 day before or after eligible date.
• Paid Vacation Time (40 hours, based upon accrual and tenure), after every year you are added an extra day of vacation up to 80 hours per year.
• Paid Sick Days (24 hours per year, based upon accrual)

Health Insurance Full-time 30 hours plus – eligible after 60 days of continuous employment:

DG Therapy Group offers pre-occupational, physical and speech therapy students the opportunity to observe individual therapy sessions, assist with group sessions, and provide clerical and equipment support to treating therapists.

Volunteers must be at least 17 years of age and adhere to dress code and confidentiality policies.

Volunteers must attend an orientation session and complete all required paperwork prior to volunteering at DGTG. Our volunteers gain a wealth of knowledge in pediatrics and their field of interest during their time here.
